Our Mission

Our mission at Hewot is to provide personalized and comprehensive care for members with chronic kidney disease, empowering them to take control of their health and improve their outcomes. By utilizing remote monitoring, care coordination, and support, we aim to slow the progression of the disease, prolong kidney function, and delay the onset of dialysis, resulting in a better quality of life for members and their families. Additionally, our approach reduces costs for payers by avoiding the need for costly interventions and promoting self-management.

Our Story

At Hewot, we understand the challenges faced by patients with chronic kidney disease (CKD) first-hand. Our founder, Mike has a lived experience with CKD and knows the importance of early intervention to slow down the progression of the disease and delay the onset of dialysis. The end user experience is often under-appreciated in chronic disease innovation. Patient innovators are better equipped to understand the needs and wants of patients. Hewot was created as a solution to help slow the advancement of kidney failure.

Hewot offers a personalized, whole-health approach to improve outcomes and reduce costs for patients with early-stage CKD or post-transplant care through remote patient monitoring and support. Our team is dedicated to addressing the needs of this underserved population.

Hewot addresses the growing epidemic of CKD and its costly impact on patients and the healthcare system through a comprehensive, personalized, whole-health approach utilizing remote patient monitoring. We are committed to improving the lives of patients with CKD and reducing the overall costs associated with the disease.

Our name, Hewot, means life in Amharic; it represents our mission of giving patients with CKD the chance to live their best lives by providing the support and tools they need to manage their condition.

Hewot understands the importance of CKD management for payers by providing cost-effective, high-quality care coordination and population health management to reduce the burden of chronic disease and comply with regulatory standards while minimizing the risk of serious complications.

Here is how Hewot can empower patients for better health outcomes and cost savings:

  • By slowing disease progression, we improve the quality of life for CKD patients while reducing long-term costs for healthcare providers and payers. Our platform helps avoid costly interventions such as dialysis and transplantation, reduces hospitalizations and other healthcare utilization. Additionally, our focus on behavior change and self-management helps patients become more active participants in their own care, further reducing healthcare costs and improving overall outcomes.

  • By addressing the physical, emotional, and behavioral aspects of CKD, our platform helps to slow disease progression, avoiding the need for more costly interventions such as dialysis and transplantation. Additionally, by coaching patients to self-manage their condition, we reduce hospitalizations and other healthcare utilization, empowering patients to become more active participants in their care and leading to better health outcomes. Furthermore, by identifying patients at high risk of progression and intervening early, we can prevent or delay progression to end-stage renal disease, which is extremely expensive and has a low quality of life.

  • Empowering patients to self-manage their early-stage CKD leads to better health outcomes as patients become more engaged in their care, leading to better adherence to treatment plans and improved disease management. Additionally, it reduces healthcare utilization, such as hospitalizations and doctor visits, which can be expensive for patients and healthcare providers. Self-management also promotes independence and autonomy for patients, improving their overall quality of life. Furthermore, it helps shift the focus of care from reactive to proactive, allowing for early intervention and preventing disease progression. Overall, empowering self-management leads to better outcomes, cost savings, and improved quality of life for patients.

  • Hewot's remote monitoring and care coordination technology allows for real-time monitoring of patient health status and timely intervention when needed. This approach improves patient outcomes, reduces hospital readmissions, and lowers healthcare costs. By proactively identifying and addressing potential health issues, healthcare providers can prevent them from becoming more serious. Additionally, Hewot's platform promotes patient engagement and self-management, empowering patients to take an active role in their care. This approach is particularly beneficial for managing chronic conditions such as CKD. It allows for early intervention and helps slow disease progression, ultimately reducing the need for more invasive and costly treatments. Overall, Hewot's remote monitoring and care coordination technology is essential for health plans to improve patient outcomes, reduce healthcare costs, and promote patient engagement.

  • Hewot's virtual patient community allows CKD members to connect with others going through similar experiences, providing support, education, and resources. Our community aims to improve the mental well-being of members by reducing feelings of isolation and allowing them to share their struggles and successes with others. Additionally, it is used to share educational materials and resources, empowering members to better self-manage their condition and avoid preventable complications. By improving mental well-being and empowering self-management, Hewot's community platform will lower healthcare costs for payers by reducing the need for hospitalizations and other healthcare utilization. Overall, a member-based community is a valuable tool for payers in improving patient outcomes and reducing healthcare costs for CKD patients.

  • Hewot addresses social determinants of health by recognizing that a member's social and economic environment plays a crucial role in their overall health and well-being. We provide whole-person personalized coaching by treating the body and mind to help address behavioral health issues, which can impact the progression of CKD. Furthermore, our virtual care approach allows us to reach members in underserved communities and provide them with the support and resources they need to manage their condition effectively, even when they may not have access to traditional in-person care. By addressing social determinants of health and empowering patients to take an active role in their care, we aim to reduce costs for payers by preventing disease progression, reducing hospitalizations, and improving overall health outcomes.

  • Hewot utilizes machine learning and AI predictive analytics to identify areas for CKD cost reduction by analyzing patient data and identifying patterns and trends that indicate potential health risks or areas for improvement. By using this technology, we are able to identify patients who may be at higher risk for progression of the disease and intervene early with personalized care plans, remote monitoring, and support. This approach helps to slow disease progression, reduce hospitalizations and other healthcare utilization, and ultimately reduce the need for more invasive and costly treatments such as dialysis and transplantation. Additionally, by identifying patterns in patient data, we can also identify areas where interventions may be most effective in improving outcomes and reducing costs, such as addressing social determinants of health. This allows us to tailor our approach to the unique needs of each patient and provide more efficient and effective care.
